Actually, I'll bet this was an error from a Temis type enrichment engine. More and more companies are using them to lock writing into template styles, and I'll bet this is a British one that replaces "The Queen" with "Queen Elizabeth II" as a style lock.
From what I remember about English Monarchy, while the Queen Mother's title was "Queen Elizabeth" (still is?), so replacing "The Queen" with "Queen Elizabeth" was incorrect from several points of view.

I suspect the human editor has software that flags all sorts of "questionable" usages, and he accidentally clicked "Replace All" rather than "No". There has been at least one famous incident of this in the Globe, "putting Washington back in the black" was turned into "putting Washington back in the African-American".
